Redeclaration of Team Size
If the number of team members changes after you have submitted your membership, you may redeclare your team size by using this form.
If you add new members to your team you must redeclare and submit a new team roster and consent forms for the new members.
If you have new team members added to your roster, please submit a new roster for your newly declared team size.
